One of the first steps in diagnosing hepatitis A involves reviewing the patient's history for potential exposure to the virus. This typically includes consumption of contaminated food or water, particularly within the past six months. Common sources include raw or undercooked seafood, unwashed fruits and vegetables, or produce washed in contaminated water. Identifying such a history helps establish a likely route of infection.

Clinical Symptoms and Presentation

Hepatitis A usually presents as an acute illness, meaning it develops quickly and lasts for a short period of time. In the early stages, many patients experience flu-like symptoms, including fatigue, low-grade fever, muscle aches, and general malaise. These early signs are often mistaken for a common cold or flu.

Progressive Symptoms

As the disease progresses, more specific symptoms related to liver function begin to appear. These may include a loss of appetite, nausea, abdominal discomfort, and dark-colored urine—often described as resembling strong tea. Additionally, jaundice may develop, causing yellowing of the skin and whites of the eyes due to a buildup of bilirubin in the bloodstream.

Lab Testing and Serological Evidence

While clinical signs and patient history are important, laboratory confirmation is essential for an accurate diagnosis. The most reliable method involves detecting specific antibodies in the blood that indicate a hepatitis A infection.

The presence of IgM antibodies against the hepatitis A virus (anti-HAV IgM) is typically the first marker to appear and is used to confirm an acute infection. In addition, a rising level of IgG antibodies over time—known as a fourfold increase in titer—can further support the diagnosis, indicating a recent or developing infection.
