After being discharged from the hospital, heart failure patients need to follow specific guidelines to maintain their health and prevent readmission. Proper care and lifestyle adjustments play a crucial role in managing this condition effectively.

Follow Medical Advice and Take Medications Regularly

Upon discharge, doctors typically prescribe a set of medications tailored to the patient's condition. It is vital to take these medications exactly as directed and attend follow-up appointments regularly. Consistency in medication intake helps stabilize heart function and prevents sudden deterioration.

Monitor Weight to Prevent Fluid Buildup

One of the most common triggers of heart failure exacerbation is fluid retention, often caused by excessive sodium intake. Monitoring body weight daily is therefore highly recommended. Patients should keep a scale next to their bed and weigh themselves first thing in the morning, before eating and after using the bathroom. A sudden weight gain of more than 4.4 pounds (2 kilograms) over a few days could signal fluid accumulation, which may require immediate medical attention and medication adjustment.
